two, Just focusing on your bow hand action on the shot, it appears that you have the pressure of the bow too high on your hand. Like the web between the thumb and forefinger taking all the pressure of the bow at full draw. That puts all the reaction there and that is above the actual center of pressure of the grip. The shot is kind of far to really see your grip but I suspect that is what is happening.
Do a search on this forum for a thread called "grip question", I posted a series of photos there that show a grip that will help with the bow wanting to tip forward. If you try this grip, the pressure of the bow will press into the base of your thumb (lower than it is now) and stop the forward tilt of your bow.
For what it is worth, the Olympic style shooters get the bow to roll forward like that but it is a planned move of the bow hand and not a reaction to having the center of pressure up in the deepest part of the grip (also called the pivot point).
One more video showing your bow hand up closer will help too.
