Here's something to build up the grip:
  Thread

You can also glue heavy tooling leather on the grip, shape it with a rasp and sandpaper, then saturate it w/ epoxy.

Stitching a wrap over a grip is in the How-to section
  How To: 1
  How to: 2

Those should get you started  :)

hey jonesy
I would try the leather first by itself. It makes a big difference. I used a piece of suede(SP) cut 1/2" wide and rolled the top so it made a stop at the shelf and started winding down the grip. used contact cement on the bottom. Works good for me. I do like the look of the one piece though..

Man I missed part of the questions..sorry

To build them up I use cork gasket material. Comes in rolls about 1/8" thick maybe? Get it at auto supply stores. Cut it with a razor or scissors. Glue on with contact cement where you need it (mulitlayer where you want like at the back) and it can be shaped with a file, then covered with leather. Another advantage is the cork absorbs some of the shock/vibration felt thru the handle. Kinda smells funny for awhile though, but it goes away.
