Personal Avatar

I saw a post here somewhere on how to get a personal avatar set up, but can't find it now that I have a picture I'd like to use. Any help would be appreciated.

If i remember correctly you need to e-mail Terry Green your real name, screen name, and member # along with the picture and a short note requesting to use it as a avatar.
