Hi Harry,
I have been using Bohning Iron insert for a couple of years, it works very well but the tip clogs easily. Now, I'm using Goat Tuff and so far so good.

I use gorilla glue "blue impact resistant".
 Works great, available everywhere and you can get them out with just heating the insert a bit.
 Never had an insert pull out yet.

Have been using crazy glue for a few years and works great. Pretty cheap to boot. Just rough up the inside of the shaft a little to get a good bond.

JB Weld or even Gorilla 5 min epoxy.
