Read Instinctive Archery Insights, by Jay Kidwell.  He has a better handle on this than anyone else I've ever heard of.

2nd Jay Kidwell's book. Best buy in archery for TP.

Yep, another vote for Kidwell's book.  I had TP and it is a very mental thing.  You've got to retrain the brain.  Mine was so bad the minute my draw hand touched my face, the shot would go off.  I did lots of drawing my bow back with an arrow nocked and not shooting.  I had to do this for about a week.  I am almost 2 years without any relapses, so it can be whooped.  Good luck and hang in there.
Always keep the wind in your face, and an arrow nocked.

Do some blank bale shooting...no target, just let the bale catch the arrow. Work on your form until it becomes automatic and consistent. When you shoot at a target, all your concentration should be on aiming. I've never had target panic, but I've had plenty of target frustration. The blank bale helped me a lot.

Watch Masters of the Bare Bow III. There are more great tips on it than one can imagine.

Blank bale with eyes closed, close to the target. Feel what a perfect shot is like and burn into memory.
