Expert opinion needed...

Hi All,
About 2 years ago, I ordered a custom bow. I finally had the opportunity to shoot it more than once here and there. During the past sessions, i have had the string come off the bow while the arrow was being released. This has happened 4 times now. At first I thought that I had somehow rigged it wrong. Since the string came back and cut my left wrist,(i'm right handed) and left a couple of welts, I made sure that the string was properly secured and centered in the notch as well as checking the brace height every single time. Still, this is the third time the string has come off, despite checking everything and even changing string. I know that there are expert bowyers on this site and want to get feedback on the notches specifically. Do you see anything wrong with one of them or both? This is getting to be frustating and its' starting to create bad shooting habits.

Need more pictures, Braced and unbraced, and a couple like this?
What is the brace Height from the deepest part of the grip to the string?
I would rather fail at something above my means, than to succeed at something  beneath my means

String grooves look uneven and not deep enough, a small round file will fix it, then fine sandpaper.

I agree with Roy.Real bad in the second pic the left side is deeper. Make them both look the same. Also the wood under the glass is compressing is what make the small cracks in it. Probably never hurt anything but needs to be rounded out more there.

Are there overlays on the back of the nocks? 

I have only seen one bow that popped it's string like that.   I acquired it new and shot it very little.   It was passed on to a grandson and he shot it for several years without problems before it started popping the string off.  It was the same string used all along.   I'm pretty sure the string creeped enough to allow the nocks to loose the string in recoil.  Could've creeped from stretch or poor construction or it may have become untwisted but it was the only thing that might have changed.   But it should not have caused the problem without some help from the nocks which did seem shallow.   So,  i made the nocks deeper all around and problem solved.
