Here is a two-arrow version I think is similar to Mickey's quiver. It's even on my Mickey Lotz selfbow from the 2005 St. Jude's Auction.

Mike at Mikes Archery Leather helped me out after I failed at getting an adequate lower piece to work.  If you are using this on a selfbow the upper and lower pieces have to be very close to the handle depending on where the limbs start to bend. That is why I usually use a single arrow solo stalker with no lower piece.

Not to hijack the threat, but Zbone, the floppy rest is all I use on my bows.  Here's a few pics:

Hope that helps!

Are these little quivers marketed nowadays?  Different variations maybe?

Now THAT is a nice idea for a selfbow.  I used a Great Northern strap on on my Owl bow this past season.  Let me first say that I have NO complaints about the Great Northern at all.  FANTASTIC piece of gear.  Absolutly excellent all the way around.  However, if you shoot selfbows, you will understand when I say that it just mars the asthetics a bit.  It doesn't "match", no matter what you do.  A floppy such as that, particularly if it were homemade, would be a fine solution.  Interesting thread.  Thanks for bringing it up.
